Brokers

Where do I find an application and/or addendum?
Applications and addendums can be found under the Applications & Addendums tab. The forms are provided in both a word and .pdf format.

What information needs to be submitted to receive a final quote?
An application, curriculum vitae (CV), prior carrier loss runs, and if applicable, detailed information about any claims and/or suits.

To whom do I submit a claim, suit, incident, or Nevada Board of Medical Examiners investigation?
Please submit this information to Michelle Morgen at michelle.morgen@ind-insurance.com or via fax at (702) 697-6401. Ms. Morgen will then take the submitted information, add a copy of the application for each individual named in the document(s), along with a copy of the applicable policy information, and submit it to our claims administrators.

When are the next IND risk management seminars?
A schedule of risk management seminars can be found here.

How do I get my name listed as an IND broker?
Please contact James Hooban at (702) 697-6402 to discuss the possibility of having your name listed as an IND broker.

How are commissions paid?
Commissions are paid after the end of an accounting month and is done on a direct bill basis.

How much in commission does IND pay?
IND pays an 8% commission.

How do I submit a loss run (claims history) request?
A written request can be submitted to Michelle Morgen at michelle.morgen@ind-insurance.com or via fax at (702) 697-6401.

What risk management options do you offer?
IND offers seminars throughout the year. For physicians who attend the seminar, a 5% premium discount is offered off their next policy renewal. An office assessment can also be performed by Idora Silver. Click here for contact information.

What type of payment plans does IND offer?
IND offers an annual pay plan and a quarterly payment plan. On accounts whose annual premiums exceed $50,000, a nine installment plan is also offered.

How do physicians and/or office staff register for the IND risk management seminars?
You can either download the registration form and mail it in with your payment or register and pay through our online process found here.

What premium discounts does IND offer?
A list of premium discounts can be found here.

Is IND rated?
IND is not yet rated by AM Best or S&P. With a solid, firm confidence in our financial standing, we will meet with you to discuss our financial details and provide copies of our audited financial statements at your request. Please contact Mr. Hooban at (702) 697-6402 to schedule a meeting.

I have received a letter from an attorney - what should I do?
Please notify your broker for any special handling and/or instructions; otherwise, please submit this information to Michelle Morgen at michelle.morgen@ind-insurance.com or via fax at (702) 697-6401. Ms. Morgen will then take the submitted information, add a copy of your application, along with a copy of the applicable policy information, and submit it to our claims administrators.

IND recommends that you utilize a broker when seeking a quote. A list of our most active brokers can be found here. IND will work with all licensed Nevada property and casualty brokers who specialize in medical professional liability insurance. All IND business is supported by licensed insurance brokers.
